ALF Financial Agreement

The price of ALF therapy with Dr. Anna Jumtina is $6000. This includes any ALF appliances (upper and lower), ALF appliance delivery, and appliance changes and repairs for the first 12 months. 

The ALF check up fee is separate. This is $250 per visit. Visit frequency will be discussed with each patient. Typically ALF check up visits are had every 6-8 weeks. Patients are responsible to come for regular check ups.

If an ALF needs repair or design changes are discussed (due to the loss of primary teeth, next phase of structural inputs desired) after the 12 months, only the lab fee will be charged. This will be discussed prior with each patient.

The ALF is made for palate expansion, jawbone remodeling, cranial structural changes and symptom improvement. If a case needs to be finished with clear aligners or (rarely) brackets, a separate fee or being seen by an orthodontist will be required. 

Myofunctional therapy or osteopathic adjustments are not included in the ALF fee but might be recommended.
